Indications and Dosage

Intra-arterial
Contrast media in magnetic resonance imaging

Adult: As a 0.002 mmol/ml solution: Dose depends on the joint being imaged; the usual range is from 1-20 ml.

Intravenous
Contrast media in magnetic resonance imaging

Adult: As a 0.5 mmol/ml solution: 0.1 mmol/kg at a rate no faster than 10 ml per 15 seconds. For cranial and spinal imaging, a further dose of 0.1 mmol/kg (up to 0.2 mmol/kg) may be given within 30 min if necessary. For whole body imaging, a dose of 0.2 mmol/kg may be needed in some cases to produce adequate contrast and, up to 0.3 mmol/kg may be used in some cases.
Child: As a 0.5 mmol/ml solution: 0.1 mmol/kg at a rate no faster than 10 ml per 15 seconds. For cranial and spinal imaging, a further dose of 0.1 mmol/kg may be given within 30 min if necessary. For whole body imaging in children >2 yr, a dose of 0.2 mmol/kg may be needed in some cases to produce adequate contrast.

Special Precautions

Patients with severe renal impairment, epilepsy, hypotension, sickle cell anaemia, children <2 yr, a history of hypersensitivity, asthma or other allergic respiratory disorders. Care should be taken to avoid extravasation. Caution when used in patients with conditions predisposed to development of thrombotic syndromes. Caution when interpreting a contrast enhanced scan in the absence of a companion unenhanced MRI.

Adverse Reactions

Headache, nausea, vomiting, paraesthesias, dizziness and localised pain. Transient sensations of heat or cold or taste disturbances on inj of gadopentetate or other gadolinium chelates. Rarely, convulsions, hypotension, allergic or anaphylactoid reactions and shock may occur.
Potentially Fatal: Severe anaphylactic reactions.

May interfere with tests of serum iron or bilirubin concentrations.

Action

Description: Meglumine gadopentetate is a paramagnetic agent that is used as a contrast medium in magnetic resonance imaging of cranial and spinal structures, the whole body and may also be used for evaluation of renal function. It is given by intra-articular injection for arthrography and has been used orally and rectally in imaging of the GI tract.
